WHAT HAPPENED?

Recently, Sleep Management Institute discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, Sleep Management Institute determined that an unauthorized actor may have accessed this sensitive information between January 27, 2024, and February 6, 2024. As a result, Sleep Management Institute began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:

  • Name
  • Social Security number
  • Date of birth
  • Address
  • Driver’s license or government-issued identification number
  • Passport number
  • Taxpayer identification number
  • Financial information (e.g., account information, payment card information)
  • Username and credential information
  • Digital signature
  • Biometric data
  • Mother’s maiden name
  • IRS-issued pin number
  • Medical information (e.g., clinical or treatment information, medical provider name(s), medical procedure information, prescription information)
  • Health insurance information
