WHAT HAPPENED?

Recently, American Premier Services discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, American Premier Services determined that an unauthorized actor likely acquired this sensitive information on or before October 13,2023. On November 29, 2023, American Premier Services began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:

  • Name
  • Social Security number
  • Date of birth
  • Driver’s license number
  • Financial account information
  • Medical information
