Formed though the merger of Surgical Associates of Washington and Orthopedic Consultants of Washington in 2002, Proliance Surgeons offers care at over 100 locations across the Puget Sound region of Washington.2 Specializing in surgery and specialty care, Proliance Surgeons provides services in orthopedics, general surgery, ENT, urology, plastic surgery, and more. Headquartered in Seattle, Washington, Proliance Surgeons consists of over 450 providers, including 200 certified physicians.

WHAT HAPPENED?

Recently, Proliance Surgeons discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, Proliance Surgeons determined that an unauthorized actor may have accessed and acquired this sensitive and protected health information on or around February 11, 2023. Recently, Proliance Surgeons began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:

  • Name
  • Social Security number
  • Date of birth
  • Driver’s license number
  • Contact information
  • Financial account number
  • Medical treatment information
  • Health insurance information
