Headquartered in Houston, Texas, Endocrine and Psychiatry Center has an additional clinic location in Katy, Texas and has 14 providers on its staff.

WHAT HAPPENED?

Recently, Endocrine and Psychiatry Center discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, Endocrine and Psychiatry Center determined that an unauthorized actor may have accessed and acquired this sensitive information sometime prior to March 20, 2023. On November 14, 2023, Endocrine and Psychiatry Center began notifying individuals whose information may have been impacted. The type of information exposed includes:

  • Name
  • Social Security number
  • Driver’s license number
  • Date of birth
  • Financial account information
  • Credit or debit card information
  • Treatment/diagnosis information
  • Health insurance information
