Founded in 1992, Deer Oaks began as a mental healthcare facility focused on treating aging adults dealing with acute disability or injury. Today, Deer Oaks provides care through over 1,500 partnered facilities across 29 states. Headquartered in San Antonio, Texas, Deer Oaks employs over 500 individuals.

WHAT HAPPENED?

On September 1, 2023, Deer Oaks discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, Deer Oaks determined that an unauthorized actor may have accessed this sensitive information through a ransomware attack on September 1, 2023. On October 31, 2023, Deer Oaks began contacting individuals whose information may have been impacted. The type of information exposed includes:

  • Name
  • Social Security number
  • Date of birth
  • Address
  • Medical information (e.g., diagnostic codes, treatment service type)
  • Health insurance information
