In order to support changing eyecare needs, Wake Family Eye Care provides comprehensive eye care for every age group.

WHAT HAPPENED?

On June 2, 2023, Wake Family Eye Care discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, Wake Family Eye Care determined that an unauthorized actor may have accessed this sensitive information on June 2, 2023. On July 14, 2023, Wake Family Eye Care began contacting individuals whose information may have been impacted. The type of information exposed includes:

  • Name
  • Social Security number
  • Address
  • Date of birth
  • Driver’s license number
  • Passport number
  • Insurance number
  • Patient records (e.g., optical images, chart number)
