Majestic Care provides a large array of medical services, including physical and speech therapy, dialysis, behavioral health assistance, hospice care, and at-home visits. Additionally, Majestic Care’s nursing facilities support residents by offering meals, social activities, medication management, and transportation. Headquartered in Westfield, Indiana, Majestic Care employs approximately 2,500 staff members throughout its network of facilities, including Majestic Care of Middletown.
WHAT HAPPENED?
On December 13, 2022, Majestic Care discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, Majestic Care determined that an unauthorized actor may have accessed this sensitive information between December 9 and December 16, 2022. On March 28, 2023, Majestic Care began contacting individuals whose information may have been impacted. The type of information exposed includes:
- Name
- Social Security number
- Date of birth
- Driver’s license number
- Address
- Contact information (e.g., telephone number)
- Medical information (e.g., healthcare payment and treatment information)
