WHAT HAPPENED?

On December 6, 2022, Southeast Colorado Hospital District discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, Southeast Colorado Hospital District determined that an unauthorized actor may have accessed this sensitive information through an employee email account. Furthermore, Southeast Colorado Hospital District concluded that this sensitive information was accessed at various times between November 23 and December 5, 2022. On February 3, 2023, Southeast Colorado Hospital District began contacting individuals whose information may have been impacted. The type of information exposed includes:

  • Name
  • Social Security number
  • Date of birth
  • Driver’s license number
  • Health insurance information
  • Medical information (e.g., treatment or diagnosis information)
