Covenant Care Data Breach Investigation

Turke & Strauss LLP, a leading data breach law firm, is investigating Covenant Care California, LLC regarding a recent data breach. The Covenant Care data breach involved sensitive personal information and protected health information belonging to an unknown number of patients who received services from Covenant Care or a facility operated by Covenant Care.

ABOUT COVENANT CARE:

Covenant Care operates skilled nursing and residential care facilities as well as home healthcare centers throughout California and Nevada. Founded in 1994 by healthcare industry professionals, Covenant Care has grown to operate over 30 healthcare and rehabilitation centers. Currently, Covenant Care employs over 8,000 healthcare professionals and cares for 4,000 residents and patients in its facilities.

Covenant Care’s headquarters is in Aliso Viejo, California. Included in Covenant Care’s family of rehabilitation centers are Wagner Heights Nursing and Rehabilitation Center in Stockton, California, and Silver Hills Health Care Center in Las Vegas, Nevada. Additionally, Covenant Care provides rehabilitation services through a company called AFFIRMA and home healthcare services under the names Focus Health, Elevate Home Health, Choice Home Health Care, and San Diego Home Health.

WHAT HAPPENED?

On or around February 24, 2022, Covenant Care discovered it had experienced a data breach in which the sensitive personal information and protected health insurance information it is systems may have been accessed. Through its investigation, Covenant Care determined that an unknown actor gained access to this information at various times between February 24 and May 3, 2022. In May 2022, Covenant Care began notifying individuals whose information may have been impacted. Recently, on June 22, 2022, Covenant Care filed additional data breach notifications with various state governments signifying that more individuals likely needed notification that their personal identifiable information and protected health information may have been impacted. The type of information exposed includes:

  • Name
  • Social Security number
  • Driver’s license number
  • Date of birth
  • Medical information
  • Health insurance information
  • Other personal information
