WHAT HAPPENED?

Recently, Valley Oaks Health discovered that it had experienced a data breach in which sensitive personal identifiable information in its systems may have been accessed and acquired. Through its investigation, Valley Oaks Health determined that an unauthorized party may have accessed this sensitive information between June 8 and June 13, 2023. On March 18, 2024, Valley Oaks Health began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:

  • Name
  • Social Security number
