WHAT HAPPENED?

On January 11, 2024, Rancho Family Medical Group was informed that its third-party technology partner, KMJ Health Solutions, had experienced a data breach in which sensitive personal information and protected health information in its systems may have been accessed and acquired. Through its investigation, KMJ Health Solutions determined that an unauthorized party may have accessed this sensitive information on or around November 19, 2023. As a result, Rancho Family Medical Group began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:

  • Name
  • Date of birth
  • Hospital medical record number
  • Hospital treatment location
  • Date of service
  • Procedure medical code
